A Daily Wire investigation has revealed that the state of Washington has funneled more than $4.3 million in taxpayer funds to a controversial "gender-affirming" surgeon who performs procedures on prison inmates. Stiller Aesthetics, led by Dr. Geoffrey Stiller, has received the massive payout from the Washington State Health Care Authority since 2019. Dr. Stiller’s practice offers various "sex-rejection" procedures, including "bottom surgery," a euphemism for the removal of male genitalia to construct a neovagina.
While President Donald Trump signed a January 2025 executive order banning federal funds for inmate sex changes, Washington State led a multi-state lawsuit to block the measure. This legal maneuver allowed the state’s Medicaid program, Apple Health, to continue covering these procedures for those behind bars. Dr. Stiller has reportedly pulled in more than $800,000 in taxpayer funds in 2025 alone, despite the administration’s efforts to "restore biological truth" to the federal government.
Dr. Stiller’s practice has been plagued by controversy, including a 2021 arrest for drunk driving and multiple medical malpractice lawsuits. A former staff member alleged that his drinking habits impaired his surgical performance, resulting in at least one botched procedure. InvestigateWest reported in December 2025 that the surgeon is also facing allegations of sexual harassment and keeping shoddy medical records.
Public records show that the "brutal realities" of these surgeries are felt by the inmates who undergo them at the public's expense. One inmate reportedly struggled with "self-image incongruence" after a mastectomy resulted in nipple placement they found unsatisfactory. Another required a single-cell placement for a full year to accommodate the painful "dilation" needed to prevent their new anatomy from closing.
In addition to his work with the prison system, Dr. Stiller has partnered with nonprofits like Genderbands to offer "FREE Top Surgery" to teens and adolescents. A 2019 social media post featured a young woman with fresh surgical scars praising Stiller for providing the procedure free of charge. Critics argue that these efforts target vulnerable youth with "life-altering" procedures that have lifelong medical consequences.
Despite the mounting legal and ethical questions, Washington continues to prioritize these surgeries as a supposed "human right" for the incarcerated. Dr. Stiller has moved his practice from Idaho to Spokane, claiming it is a "safer" environment to care for gender-confused patients. As the battle between the Trump administration and far-left states intensifies, taxpayers in Washington remain on the hook for millions in "sex-rejection" spending.
